Huntsville Memorial Hospital’s Diabetes Self-Management (DSM) Program increases patient awareness of the diabetes triggers while providing education about preventing serious complications. The DSM program is conducted by a Certified Diabetes Educator and a Registered Dietitian.
The goal is to assess each patient’s condition and develop an individualized care plan. The program includes a series of individual consultations and progress report updates for the referring physicians. The program includes nutrition, meal-and-exercise planning, blood glucose monitoring and medication management. This program requires a physician referral.
