Diabetes Self-Management
Diabetes is a serious disease that can affect your entire body. However,
through recognition of symptoms and disease education, it is a condition
that can be controlled. Huntsville Memorial Hospital's Diabetes Self-Management
(DSM) Program increases patient awareness of the triggers of their condition
while educating them on how to prevent serious complications. The DSM
program is conducted by a Certified Diabetes Educator and a Registered
Dietitian.
The goal is to assess each patient’s condition and develop an individualized
care plan. The program includes a series of individual consultations and
progress report updates for the referring physicians. Areas of the program
include nutrition, meal and exercise planning, blood glucose monitoring
and medication management. This program requires a physician referral.
Huntsville Memorial Hospital's outpatient dietitian develops an individual
care plan for each patient. Whether you are newly diagnosed or have had
diabetes for years, the DSM program can give you the skills to improve
your quality of life.
