The Patient and Consumer Health Portal is an online tool that goes a step
beyond simply displaying information by helping you manage your healthcare
easily and securely.

You will need your personal medical record number, your birth date, and
an e-mail address. Visit
my.huntsvillememorial.com and follow directions to set up an account.
Upon entering your information, you will receive an e-mail with a temporary
log-in user identification code. With this code, you will log in again,
and create your permanent log-in user identification code and password.
Please store this information in a private place for it is personal to
your healthcare records.
To create an account, you will need your:
Medical Record Number
All patients of Huntsville Memorial Hospital will receive their personal
medical number at the time of admission or discharge from services. If
an additional card is needed or an individual would like to enroll as
a patient portal user, then a personal medical record number may be obtained
by visiting the Health Information Management Department at Huntsville
Memorial Hospital (936) 291-4270. Proof of identification with a picture
Patients may access their personal electronic medical record at any time
and from anywhere through the Huntsville Memorial Hospital website.
Access includes the patient's demographic information, a medical record
summary and test results for those performed at our hospital. Parents
may also access medical records of their family dependents.
Once logged in, patients can view:
- Lab Results
- X-Ray Results
Patient records are private and downloadable. The patient may change their
personal contact information and download a copy of their healthcare information.
Patient Portal records only include medical record summary, test results,
and medications given at Huntsville Memorial Hospital. List of medications
only includes those electronically entered and/or prescribed at patient's
discharge. Patient Portal records are for the patient's information
only. Any questions regarding test results should be directed to the patient's
primary care physician's office
Patient e-mail addresses are kept confidential and are not shared with
any third parties (unless it is required by law or necessary to carry
out our services).
