• You must require a skilled, intermittent service such as nursing care, physical therapy, or speech therapy.
• You must be essentially homebound due to an illness/injury which makes it difficult for you to leave home in order to receive medical care.

• Skilled Nursing Care
  Care may include but is not limited to:
  • Assessment of condition
  • Dressing changes
  • Wound care
  • Patient education
  • Catheter care and insertion

• Personal Care
  Care may include but is not limited to:
  • Baths
  • Personal-care related services
  Note: Medicare patients must be receiving a skilled service in order to be eligible for personal care.

  Specific therapy may include but is not limited to:
  
  • Exercise to strengthen muscles
  • Assistance with walking and transferring from chair to bed
  • Assistance with activities of daily life, e.g., dressing, eating, toileting
  • Exercise to help talk and swallow
  
  
• Medical Social Services
  Care may include but is not limited to:
  • Counseling and coordination of community and financial resources.
  • Assistance coping with social and emotional problems which often accompany illness.
  
  
• Medical Supplies and Equipment
  
  • Includes assistance with the arrangements, supplies and equipment needed to meet the patient's individualized needs.
